Please be prepared to upload the following information from you provider.

1. MMR vaccine is required for everyone born after Dec. 31, 1956. Two doses are required for entry into the state university system of Florida. First dose must have been received after the 1^st birthdate. The second dose must have been received at least 30 days after the first dose or provide lab evidence of immunity.
2. Hepatitis B Vaccine is required or provide lab evidence or immunity by doing a blood test or sign a waiver which indicates you understand the possible risk in not receiving this vaccine.
3. Menactra/MCV4 (Meningococcal Meningitis Vaccine) Students are required to provide proof of the meningitis vaccine, one dose after 16^th birthday or sign the waiver which indicates the possible risk of not receiving the vaccine.ÌýÌýÌýÌýÌýÌýÌýÌýÌýÌýÌý
4. Tuberculosis Screening is required if TB screening questionnaire is positive, or a student has lived in a high-risk country for 6 months or more.
